About the Role
Spectrum Support Services is seeking a dedicated Physical Therapist to provide rehabilitative and preventative care to adults aged 21 and older with developmental and intellectual disabilities across Bergen County, NJ.
This role is distinct from pediatric school-based PT — you'll be working with adults in community, in-home, and clinic settings, developing individualized treatment plans that support long-term mobility, independence, and quality of life. Familiarity with NJ DDD Individual Service Plans is a real advantage here.
What you'll be doing:
Evaluating adult clients and developing individualized physical therapy treatment plans
Delivering PT services in community, in-home, and clinic-based settings across northern NJ
Coordinating with NJ DDD case managers, physicians, and other care providers
Educating clients and caregivers on home exercise programs and injury prevention
Maintaining documentation compliant with NJ DDD and Medicaid requirements
Requirements
Evaluate clients and develop individualized treatment plans based on physical therapy goals.
Deliver direct physical therapy interventions in clinic, in-home, or community-based settings.
Educate clients and caregivers on techniques for continued progress and injury prevention.
Coordinate with physicians, case managers, and other healthcare providers.
Maintain clear, compliant documentation of progress and treatment.
Licensed Physical Therapist
Degree in Physical Therapy (Doctorate or Master's)
Experience working with adult and/or geriatric populations preferred
Strong interpersonal, documentation, and problem-solving skills
Familiarity with DDD requirements and Individual Service Plans
