About the Role
We are seeking a dedicated and skilled Physical Therapist to provide rehabilitative and preventative care to adults aged 21 and older. You will play a critical role in helping clients regain physical function, manage pain, and improve mobility and quality of life in a supportive, client-focused environment.
Requirements
Evaluate clients and develop individualized treatment plans based on physical therapy goals.
Deliver direct physical therapy interventions in clinic, in-home, or community-based settings.
Educate clients and caregivers on techniques for continued progress and injury prevention.
Coordinate with physicians, case managers, and other healthcare providers.
Maintain clear, compliant documentation of progress and treatment.
Licensed Physical Therapist
Degree in Physical Therapy (Doctorate or Master's)
Experience working with adult and/or geriatric populations preferred
Strong interpersonal, documentation, and problem-solving skills
Familiarity with DDD requirements and Individual Service Plans
Focus on quality, patient-centered care
